Care Coordination offer culturally sensitive informational counseling to individuals with chronic, mental health illnesses and/or homelessness.  Dennelisse LHCSA intensive Care Coordination program was formerly a COBRA Case Management service that focused on improving the quality and wellbeing of those individuals and their collaterals living with HIV/AIDS.

The Care Coordination Team assists with housing, entitlements, accessing quality healthcare, as well as specialty health care providers, while being person-centered.  Embedded in the programs objectives are to reduce the number of Emergency Room visits and hospital admissions.

Dennelisse LHCSA partners with various hospitals, and provider networks throughout the City of NY in order to ensure optimal outcomes.
