Home Health services are provided for those who desire to remain in their homes rather than enter a skilled care facility. Our team of registered nurses, social workers and certified paraprofessionals work in collaboration with the personal physician to enhance compliance with prescribed medical regime, assist with activities of daily living, and maximize personal independence. Services include activities and support unique to each family’s circumstances. Services may be temporary or long term depending upon an individual’s medical, or mental status and/or rehabilitation or recuperation. Our specially trained staff will ensure that you, or your loved ones, are able to safely live as independently as possible in the comfort of familiar surroundings. A paraprofessional is supervised by a dedicated case coordinator and a registered nurse. A registered nurse evaluates needs, ensures the scope of service is being met under the individual’s physician care plan and that the healing process is as comforting as possible.
DENNELISSE LHCSA assists families with a life balance that reduces the stress of caring for a loved one while in the hospital, upon discharge or a loss of memory. Our trusted solution enables loves ones to enhance their quality of life and establish a regular routine in their familiar surroundings.
